Why do optical transceiver modules burn out?

This is common in long-distance transmission modules: when connected to very short-distance optical fibers, the received optical power may far exceed its overload power, causing the optical detector to burn out. Accidentally connecting the OTDR to a working optical transceiver module during testing can also burn out the module.
